To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by: (1) mail: U.S. Department of Agriculture, Office of the Assistant Secretary for Civil Rights, 1400 Independence Avenue, SW, Washington, D.C. 20250-9410; (2) fax: (202) 690-7442; or (3) email: program.intake@usda.gov.

USD 330 Mission Valley is a rural 2A school district in NE Kansas that seeks to provide a high quality education. Mission Valley has an average enrollment of 450 students and a total staff of 130 employees. The district is comprised of 372 square miles in Wabaunsee, Shawnee, Osage, and Lyon counties. USD 330 Mission Valley is a member of the Flint Hills League and offers a wide range of athletic programs including football, volleyball, cross-country, basketball, wrestling, track, baseball and softball. In addition, opportunities for students to showcase their talents in Fine Arts {Forensics, Band, Vocal Music, Art}, as well as, outstanding programs/clubs/organizations such as FCA, FCCLA, FBLA, FFA, Kays, Scholar’s Bowl, and National Honor Society abound.  Many of these programs achieve recognition at the state and national level on an annual basis.
